New Patient Registration
If you wish to register please complete the online registration forms found below! (To be eligible to register at the practice you must live within the practice catchment area, you can check this below).
A member of the team will send you an appointment for your new patient health check this will be conducted via a video link or over the phone once your registration is active.
If you are on medication or need to see the GP we may ask you to attend the surgery. Please note you will be screened for COVID prior to your appointment. If you develop any symptoms please let us know and do not attend the surgery.
If you are unable to complete this electronically please let us know and we can arrange for you to pick up a paper registration pack.
Do you live in the catchment area?
To be eligible to register at the practice you must live within the practice catchment area.
Please check that your road is included in the catchment area. You can by using the map below.
If you submit a registration form and are not living within the practice catchment area, your registration will not be processed.

Note that by completing the link you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively, you may print off a registration form, fill it out and bring it in with you on your first visit to the practice (please call ahead so we can complete a covid assessment before coming into the practice).
If you would like to register in person, please come to the surgery on Monday to Friday, between 9am and 6pm.
Accepting and Removing Patients from Our List
We will normally accept any patient living within our practice area onto our list. If you move out of our area, we will ask you to re-register with a more local doctor. In exceptional circumstances, we reserve the right to keep or remove any patient from our list.
Named Accountable GP
From 1 April 2015, practices are required under the GMS contract, to allocate a named accountable GP to all patients, including children. All patients registered before 1 April 2015 will be allocated a GP before the end of June 2015.
The named GP will have overall responsibility for your care. You will be sent either a SMS or email informing you of your Named GP once you are registered
