New Patient Registration
If you wish to register please complete the online registration forms below. A member of the practice team will be in touch.
They will send you a link via SMS for you to upload your documents to. Alternatively you can email them to CAHCCG.email@example.com
We will review your proof of address and proof of identity.
We will accept the following as proof of address:
- Council Tax Bill
- Utility Bills [not mobile phone bills]
- Bank Statement [no older than three months]
- Tenancy Agreement/Rent Book
A member of the team will send you an appointment for your new patient health check this will be conducted via a video link or over the phone.
If you are on medication or need to see the GP we may ask you to attend the surgery. Please not you will be screened for COVID prior to your appointment. If you develop any symptoms please let us know and do not attend the surgery.
If you are unable to complete this electronically please let us know and we can arrange for you to pick up a paper registration pack.
Accepting and Removing Patients from Our List
We will normally accept any patient living within our practice area onto our list. If you move out of our area, we will ask you to re-register with a more local doctor. In exceptional circumstances, we reserve the right to remove any patient from our list.
